What is the cost of membership? The annual membership fee varies depending on your individual circumstances and needs. Please call our office for more details at 617-777-4080.

What does the annual fee cover? Our annual fee gives you access to an unsurpassed level of quality care and an unwavering commitment to meeting your individual health needs. In fact, the care Dr. Costa provides is custom tailored and 100% driven by the needs of each patient he serves. For example, if you need extra time to discuss and address medical issues, Dr. Costa will provide that. If you are a busy executive and need to manage aspects of your healthcare by phone, email or through coordination with international medical facilities, Dr. Costa is fully prepared to do that.  If you have a particular specialist or healthcare facility that you would like to maintain a relationship with, we will help you preserve that.

Your annual fee also covers a complete preventive care and health optimization plan using traditional Western medicine as well as alternative non-pharmacologic therapies such as nutritional supplements, acupuncture, massage and craniosacral therapy. Dr. Costa uses his expertise to address everything from arthritis, heart disease, diabetes and cancer to stress, sleep disturbances, nutritional issues and more. And if you need specialized care, you’ll have the confidence of knowing that Dr. Costa only refers to highly trained specialists whom he knows personally and trusts for his own healthcare needs. As a member of Enhanced Medical Care and a patient of Dr. Costa, you can expect care that truly meets your unique needs and exceeds all your expectations.

Will I still need my health insurance? Yes, you will still need your health insurance to cover visits to my office, specialists, hospitals, and other essential healthcare services. Although the annual fee for my services is not covered by insurance, many people have found that membership fees are reimbursable under their company's medical reimbursement, 125, or cafeteria plan.

You have specialized training in heart disease. Are you still a primary care physician?

Yes. I am a 100% primary care physician.

Are you really available all the time – including nights and weekends? 
Practically, yes, I am. That’s part of what your membership covers. You can even arrange a house call with me if your situation does not require diagnostic equipment. Another option: I can visit you at your office if you are unable to get to me. 



How do you handle prescriptions? 
On the same day of your request, we will promptly transmit electronically your prescription to your pharmacy or a convenient 24-hour pharmacy near you. This system is highly efficient. Our patients love having their medications waiting for THEM at the pharmacy.

What if I have a medical emergency? 
Dial 911, then call me.
 Call my personal number as soon as you have called 911 or visited the local emergency room. I can provide important personal information from my records that will be helpful to your emergency medical staff. I will also be able to schedule follow-up visits, or, attend in person to assist in developing a plan of treatment.


Do you take vacations?
 Yes, but you are still covered. I plan on four weeks of vacation during the year, but will always give you advance written/email notice of my plans and dates. During that time, I will be available for telephone consultations but not face-to-face visits. A distinguished colleague of mine and membership practice physician covers my practice when I am on vacation.

What if I need a specialist? 
I will facilitate streamlined arrangements with Boston’s finest  doctors, medical and surgical specialists for you. Of course, I will always participate in important consultations with specialists to help you understand and make wise health care decisions.  

Will I be able to access my medical records? 
Absolutely. You always have the right (with prior written notice) to access your medical and billing records so you can always inspect or request copies of your records. Of course, Enhanced Medical Care, LLC will always maintain confidentiality of your medical records as required by law.
 
Do I have to become a member to see you?
 Your first visit is free, but after that, membership in Enhanced Medical Care requires the payment of an annual fee. Membership fees are solely for the member benefits provided, and do not cover additional clinical care provided by Dr. Costa or his practice, Enhanced Medical Care, LLC, or any items or services covered by the member's health plan.

What if I want to terminate my membership? 
No problem. You may terminate your relationship with Enhanced Medical Care for any reason. Just put it in writing to me and it shall be effective upon receipt. I reserve the right to terminate our relationship  if you or someone acting on your behalf were to abuse Enhanced Medical Care services. I will provide written notice of my termination, which will be effective thirty days after your receipt. In the event of termination by either party, I will refund a prorated portion of your annual fee. You will receive your refund within 30 days of the date of termination minus a $250 administration fee.
